1. You are a current member of a Pacific University intercollegiate athletic team.

2. You have received a pre-participation medical examination and the proper form is completed. NCAA and Pacific University Athletic Department guidelines stipulate that this be accomplished upon entrance into the athletic program. ONLY NEW/TRANSFER STUDENT-ATHLETES ARE REQUIRED TO GET A NEW PHYSICAL. RETURNING STUDENT-ATHLETES ARE NO LONGER REQUIRED TO RECEIVE A PHYSICAL EACH YEAR, ONLY FILL OUT THE NEW MEDICAL HISTORY FORM AS PART OF THEIR CLEARANCE PAPERWORK.

3. All updated and current medical forms are on file with the Sports Medicine staff. Current forms will be available via Docusign.

There may be instances in which an athlete may need referral to one of the Pacific Sports Medicine Team Physicians or outside physicians. These visits are determined and scheduled, if necessary, following a clinical evaluation by a staff ATC. Private practice office visits with the team physician will incur charges and be billed to the patient or the primary insurance carrier.

Pacific University's Head Team Physician, Dr. Brian Bettencourt, will be the final authority as to whether a Pacific University student-athlete can safely return to practice and/or play, even if a student-athlete has sought another medical opinion.
